Wow - the last month of the year! Where did 2006 go? Here’s the December e-newsletter! I do hope you enjoy these tidbits and please let me know how I can help you be even more successful in your quality improvement activities. 1. The updated information on the quality measures has been posted to the Nursing Home Compare website and made available to the public. These data will reflect care delivered to post-acute care nursing home residents between January 1, 2006 and June 30, 2006, and to chronic care residents between April 1 and June 30, 2006. To see it, go to: www.medicare.govALSO Last year 50% of the facilities in Texas set their targets for quality improvement. Have you done it this year? It’s Time to SET YOUR TARGETS!!! Go to the STAR website at: www.nhqi-star.org to set your targets for they 4 focus quality measures. You will be setting your goal for the year in how you want to improve care in your facility! Call me if you have any questions, but it takes only about 20 minutes to set up your account and get the 4 measures set. One benefit is graphs! You can see and print graphs, in color if you wish, and see how you are doing and what your projection is for improvement. It's a great way to motivate your staff and to help them see the impact they are having on your improvement efforts!Texas is below the national average in: 1. Restraints @ 5.3% vs Nation @ 6.3%! (top 20% of the facilities in the country are @ 0%) 2. High-Risk Pressure Ulcers @ 12.3% vs Nation @ 12.8% (top 10% of the facilities in the country are @ 4.4%) 3. Depression @ 8.5% vs Nation @ 14.3% (top 10% of the facilities in the country are @ 4.4%) 4. Pain @ 4.6% vs Nation @ 5.1% (top 10% of the facilities in the country are @ 0.6%) If you need some help with your quality improvement activities, refer to our website at www.tmf.org and look at our toolkits for the focused measures!2. On November 3rd CMS broadcast a two and a half hour presentation on the topic of Integrating Individualized Care and Quality Improvement. This is Part One of a four part series: From Institutional to Individualized Care. The goal of this broadcast is to provide a framework and practical examples to help LTC surveyors, providers, and consumers understand and support individualized care. Restraint Update : Go to the DADS Provider letter of 6/06, pages 19 & 20 and you’ll find new posting requirements. You must now post your restraint and seclusion policy.
