MDS Navigator® 3.0  - CP9119SP


The MDS Navigator was created to help responsible staff navigate the maze of OBRA and Medicare required MDS assessments.  This new version of the popular MDS Navigator 3.0 has been updated to provide a great tool for the new and experienced MDS Coordinator, but especially for those still learning MDS 3.0 assessment.


  1.    Side One provides a calendar wheel for determining MDS assessment dates including ARD, MDS     completion, CAA completion and Care Plan completion dates.


  1.    Side Two highlights Medicare scheduled and unscheduled assessments with essential coding and timeframes for each.



ADL Tracker - Single Entry - CP3024

 

This convenient pocket-sized form is designed for use by CNAs to capture crucial ADL supporting documentation required to accurately code the MDS 3.0 and ensure appropriate reimbursement.


This version documents in a single entry the number of staff assisting and degree of assistance for each ADL occurrence on a shift.


Use one form per resident per shift to reduce the likelihood of "copy-cat" charting associated with daily/monthly charting forms.


It is recommended that completed ADL Tracker forms be kept in a notebook until the MDS assessment is performed. At the time of the assessment remove the pages of pertinence and place them in the active clinical record for supporting documentation while filing the remaining in Medical Records to minimize the number of pages on the unit. The correct use of these forms can minimize collecting the wrong data and help ensure receiving payment that has been earned.

Weekly / Monthly Care Summary - CP3205


Updated to reflect MDS 3.0 including documentation sections for mood and behavior.


This four-page form documents a review of MDS PPS items. The form complements MDS documentation by following the order of the MDS 3.0 item Set and using the same language and coding.


Note: Can be used to capture information critical for state reimbursement where the state is utilizing RUG 34, 44 or 53 for Medicaid.
